The Health Passport, first launched in 2011 by HDC, was adapted from a similar resource in the UK (hospital passport), which had been developed for use by people with learning disability while they were in hospital.
In 2020, HDC worked with the Strategy, Innovation and Performance Disability Responsiveness team of the three District Health Boards (DHBs) in the Lower North Island (Capital & Coast DHB, Hutt Valley DHB, and Wairarapa DHB), disabled people, and people who provide health and disability services, to revise the Health Passport and make it work for everybody.
The revised Health Passport is called My Health Passport, and is available in a number of formats.
